Which Documents Are Required for a Medical Flight

When arranging a medical flight—whether transporting a patient back to Israel or from Israel to a hospital abroad—it is essential to prepare the required documents in advance.
At Medassis, we provide full bed-to-bed medical transport, handling every stage of the process, including documentation, medical approvals, airport coordination, and communication with hospitals and insurance providers.
Below is a clear checklist that helps ensure a fast, safe, and smooth medical evacuation:

Identification Documents
• Patient’s passport, and passports of any accompanying family members — required for official identification and coordination with airports, airlines, and customs.
• Entry permits or visas for the destination country (when necessary) — especially relevant for international air ambulance flights.
• Authorization from a family member or legal guardian, if the patient is unable to sign documents on their own.

Medical Documents
• Current medical report from the treating physician, including diagnosis, treatment history, and physical condition (Hebrew or English).
• Recent medical tests, such as blood work, imaging reports, surgical reports, oxygen saturation, or respiratory data — depending on the patient’s condition. These documents help determine what medical equipment and monitoring will be required during the flight.
• “Fit to Fly” approval — a medical certificate confirming the patient is stable enough for transport by medical flight.
• List of current medications, allergies, and chronic treatments so the on-board medical escort team can provide proper care throughout the journey.
• Hospital discharge form, in some cases, if the patient is being released directly into the care of the medical transport team.

Why preparing documents matters
Proper documentation is critical for a successful air medical transport:
• Prevents delays — once documents are ready, the medical evacuation can begin quickly.
• Ensures medical safety — advance information allows us to assign the correct aircraft, medical escort, and equipment.
• Supports the family — early preparation reduces stress and improves coordination between relatives, hospitals, and the medical flight provider.
• Protects the patient — with a full medical history, the crew can respond quickly and avoid risks during the flight.
